Disruptions in the circadian rhythm are factors in developing cardiovascular disease, cancer, anxiety and ageing. A feedback loop is a key mechanism regulating the molecular circadian clock. Per3 is a transcriptional repressor and also regulates temporal organisation in peripheral tissues. Per3, with other proteins, forms the negative pathway of the feedback loop; Per3 interacts with the NPAS2-BMAL1/BMAL2 heterodimer inhibiting transcriptional activity and thereby negatively regulating Per3’s own expression. Per3 is one of the most robustly rhythmic expressed genes.

Per3 has a redundant role with the other Per proteins Per1 and Per2 and is not essential for maintaining the circadian rhythm. However, Per3 is vital in sleep periods through transcriptional regulation of sleep homeostasis-related genes. Polymorphisms appear to influence sleep regulation and circadian preference. Other polymorphisms have been linked to increased anxiety.
